Forms You Will Need to Apply
You may download an Application for Admission. The Application for Admission form should be fully completed and mailed to the Office of Admissions along with the appropriate application fee.
You may download the required Pastor/Church Recommendation form. The form may be printed, completed by the appropriate person, and mailed to the Office of Admissions.
You may download the required Character Recommendation form.The form may be printed, completed by the appropriate person, and mailed to the Office of Admissions.
You may download the required Church Membership Certification form.The form may be printed, completed by the appropriate person, and mailed to the Office of Admissions.
You will need to request an official high school transcript, which should be sent directly to the Seminary. Additionally,official transcripts of all college or graduate work you would like evaluated by the Office of Academic Affairs. For your convenience, we are providing you a Transcript Submission form which may be downloaded. The form may be printed, completed and mailed to the appropriate educational institution along with any fee the institution may require. The receiving institution will mail the official transcript to our Office of Admissions.
